Dosing
Mantle cell lymphoma 200 mg orally once daily until disease progression or unacceptable toxicity; swallow tablets whole with water, with or without food.
Chronic Lymphocytic Leukemia, Small Lymphocytic Lymphoma 200 mg orally once daily until disease progression or unacceptable toxicity; swallow tablets whole with water, with or without food.
Chronic Lymphocytic Leukemia 100 mg/m2 IV over 10 minutes on Days 1 and 2 of a 28-day cycle, up to 6 cycles.
Indolent B-Cell Non-Hodgkin Lymphoma 120 mg/m2 IV over 10 minutes on Days 1 and 2 of a 21-day cycle, up to 8 cycles.
Contraindications

- Known hypersensitivity (e.g., anaphylactic and anaphylactoid reactions) to bendamustine, polyethylene glycol 400, propylene glycol, or monothioglycerol

Pharmacology
Pirtobrutinib is a small molecule, noncovalent inhibitor of BTK, a signaling protein of the B-cell antigen receptor and cytokine receptor pathways; it binds wild-type BTK and BTK harboring C481 mutations to inhibit BTK kinase activity and suppress malignant B-cell proliferation.
Bendamustine is a bifunctional mechlorethamine derivative containing a benzimidazole ring that forms electrophilic alkyl groups, which create covalent interstrand DNA crosslinks leading to cell death via multiple pathways; it is active against both quiescent and dividing cells.
