Dosing
Diabetes Mellitus, Type 1 Approximately one-third of total daily insulin requirements SC once daily at the same time each day; use concomitantly with short-acting premeal insulin.
Diabetes Mellitus, Type 2 0.2 units/kg or up to 10 units SC once daily at the same time each day for insulin-naive patients; individualize based on metabolic needs and glycemic goals.
Diabetes Mellitus Administer subcutaneously within 15 minutes before or immediately after a meal into the abdominal wall, thigh, upper arm, or buttocks; HUMALOG U-100 may also be administered by continuous subcutaneous infusion via insulin pump or by intravenous infusion (diluted, under medical supervision); dosage must be individualized based on route, metabolic needs, and blood glucose monitoring results.

Pharmacology
Insulin glargine-yfgn is a long-acting human insulin analog that lowers blood glucose by stimulating peripheral glucose uptake in skeletal muscle and fat and inhibiting hepatic glucose production; it also inhibits lipolysis and proteolysis and enhances protein synthesis.
Insulin lispro is a rapid-acting human insulin analog that lowers blood glucose by stimulating peripheral glucose uptake by skeletal muscle and fat and inhibiting hepatic glucose production; it is equipotent to human insulin on a molar basis but has a more rapid onset and shorter duration of action.
