Dosing
Menopausal syndrome, Osteoporosis, Postmenopausal 0.045 mg estradiol/0.015 mg levonorgestrel per day applied transdermally once weekly; for vasomotor symptoms, attempts to discontinue should be made at 3 to 6 month intervals.
Menopausal syndrome Start with 0.5 mg estradiol/100 mg progesterone orally once each evening with food; adjust dose based on clinical response; attempt to taper or discontinue at 3 to 6 month intervals.
Contraindications
- Undiagnosed abnormal genital bleeding
- Breast cancer or history of breast cancer
- Estrogen-dependent neoplasia
- Active DVT, PE, or a history of these conditions
- Active arterial thromboembolic disease (e.g., stroke or MI), or a history of these conditions
- Known anaphylactic reaction, angioedema, or hypersensitivity to Climara Pro
- Hepatic impairment or disease
- Protein C, protein S, or antithrombin deficiency, or other thrombophilic disorders
- Abnormal genital bleeding of unknown etiology
- Breast cancer or history of breast cancer
- Estrogen-dependent neoplasia
- Active deep vein thrombosis (DVT), pulmonary embolism (PE), or history of these conditions
- Active arterial thromboembolic disease (stroke, myocardial infarction) or history of these conditions
- Known anaphylactic reaction, angioedema, or hypersensitivity to BIJUVA
- Hepatic impairment or disease
- Known thrombophilic disorders, such as protein C, protein S, or antithrombin deficiency

Estrogen/progestogen combination; estradiol binds nuclear estrogen receptors in estrogen-responsive tissues to reduce elevated postmenopausal gonadotropin levels, while levonorgestrel inhibits gonadotropin production and counteracts the proliferative effects of estrogen on the endometrium.
