Dosing
Migraine Disorders 240 mg loading dose (two consecutive SC injections of 120 mg each) once, followed by 120 mg SC monthly.
Cluster Headache 300 mg (three consecutive SC injections of 100 mg each) at the onset of the cluster period, then monthly until the end of the cluster period.
Migraine Disorders 75 mg orally as needed; maximum 75 mg per 24-hour period; safety of more than 18 doses in a 30-day period has not been established; may be taken with or without food.
Episodic migraine 75 mg orally every other day, with or without food.
Contraindications
  • Serious hypersensitivity to galcanezumab-gnlm or any of the excipients
  • History of hypersensitivity reaction to rimegepant, NURTEC ODT, or any of its components, including anaphylaxis and delayed serious hypersensitivity

Pharmacology
Galcanezumab-gnlm is a humanized monoclonal antibody that binds to calcitonin gene-related peptide (CGRP) ligand and blocks its binding to the receptor; it is not metabolized by cytochrome P450 enzymes, making interactions with CYP substrates, inducers, or inhibitors unlikely.
Rimegepant is a calcitonin gene-related peptide (CGRP) receptor antagonist; it is an orally administered small molecule that blocks the CGRP receptor implicated in migraine pathophysiology, indicated for both acute and preventive treatment.
