Dosing
Primary hypogonadism, Hypogonadotropic hypogonadism, Klinefelter Syndrome 750 mg (3 mL) IM injection deeply into the gluteal muscle at initiation, repeated after 4 weeks, then every 10 weeks thereafter; observe patients for 30 minutes after each injection.
Primary hypogonadism, Hypogonadotropic hypogonadism 225 mg (two 112.5 mg capsules) orally twice daily with food, once in the morning and once in the evening; monitor serum testosterone 8-9 hours after morning dose at 3-4 weeks and periodically thereafter.
Contraindications
  • Carcinoma of the breast or known or suspected carcinoma of the prostate
  • Pregnancy (testosterone causes virilization of the female fetus)
  • Known hypersensitivity to AVEED or any of its ingredients (testosterone undecanoate, refined castor oil, benzyl benzoate)

Adverse Reactions
Most common (>2%) Acne, injection site pain, prostatic specific antigen increased, estradiol increased, hypogonadism
Serious Pulmonary oil microembolism (POME), anaphylaxis, blood pressure increases, cardiovascular events including non-fatal arrhythmias, atrial fibrillation, and acute kidney injury
Postmarketing Polycythemia, thrombocytopenia, myocardial infarction, stroke, deep venous thrombosis, pulmonary embolism, prostate cancer, gynecomastia, hepatic function abnormalities
Most common (>=2%) increased blood prolactin, hypertension, increased hematocrit, upper respiratory tract infection, weight increased, headache, musculoskeletal pain
Serious polycythemia, venous thromboembolism, worsening of benign prostatic hyperplasia and potential risk of prostate cancer, blood pressure increases, hepatic adverse effects, edema, sleep apnea, gynecomastia, lipid changes, hypercalcemia

Pharmacology
Testosterone replacement agent; endogenous androgen responsible for normal growth and development of male sex organs and maintenance of secondary sex characteristics including hair distribution, laryngeal enlargement, and body musculature and fat distribution.
Androgen; testosterone undecanoate is a lipophilic ester of testosterone that, after oral absorption primarily via the lymphatic system (bypassing first-pass hepatic metabolism), is converted to testosterone, which is responsible for normal growth and development of male sex organs and maintenance of secondary sex characteristics.
