Indications

Dosing
Pulmonary arterial hypertension Starting dose 0.125 mg TID or 0.25 mg BID with food; titrate by 0.125 mg TID or 0.25-0.5 mg BID no more frequently than every 3-4 days; maximum daily dose 120 mg.
Pulmonary arterial hypertension Adults and patients >12 years and >40 kg: 62.5 mg orally twice daily for 4 weeks, then 125 mg twice daily; patients >12 years and <=40 kg: 62.5 mg twice daily; pediatric patients <=12 years: weight-based dosing ranging from 16 mg to 64 mg twice daily.
Contraindications
- Severe hepatic impairment (Child Pugh Class C)
- Pregnancy
- Concomitant use with cyclosporine A
- Concomitant use with glyburide
- Hypersensitivity to bosentan or any component of the product
Adverse Reactions
Most common (>10%) Headache, diarrhea, nausea, vomiting, jaw pain, flushing
Postmarketing Dizziness, dyspepsia, vomiting, myalgia, arthralgia
Most common (>=3%) Respiratory tract infection, headache, edema, chest pain, syncope, flushing, hypotension, sinusitis, arthralgia, abnormal serum aminotransferases, palpitations, anemia
Serious Hepatotoxicity, embryo-fetal toxicity, fluid retention, decreased sperm counts, decreases in hemoglobin and hematocrit
Postmarketing Angioedema, unexplained hepatic cirrhosis, liver failure, hypersensitivity/DRESS/anaphylaxis, thrombocytopenia, rash, jaundice, anemia requiring transfusion, neutropenia, leukopenia, nasal congestion, autoimmune hepatitis
Pharmacology
Prostacyclin mimetic; treprostinil causes direct vasodilation of pulmonary and systemic arterial vascular beds, inhibits platelet aggregation, and inhibits smooth muscle cell proliferation.
Bosentan is a specific and competitive antagonist at endothelin receptor types ETA and ETB, blocking the vasoconstrictive and proliferative effects of endothelin-1, a neurohormone elevated in plasma and lung tissue of patients with PAH.
