Dosing
Von Hippel-Lindau Syndrome Adults: 120 mg orally once daily. Pediatric patients >=12 years: 120 mg orally once daily if >=40 kg or 80 mg orally once daily if <40 kg.
Renal Cell Carcinoma Adults: 120 mg orally once daily.
Pheochromocytoma Adults: 120 mg orally once daily. Pediatric patients >=12 years: 120 mg orally once daily if >=40 kg or 80 mg orally once daily if <40 kg.
Paraganglioma Adults: 120 mg orally once daily. Pediatric patients >=12 years: 120 mg orally once daily if >=40 kg or 80 mg orally once daily if <40 kg.
Melanoma (unresectable or metastatic) 3 mg/kg IV every 3 weeks for 4 doses, or 3 mg/kg IV with nivolumab 1 mg/kg IV every 3 weeks for 4 doses followed by nivolumab monotherapy
Melanoma (adjuvant) 3 mg/kg IV every 3 weeks for 4 doses, then 3 mg/kg every 12 weeks for up to 4 additional doses
Renal Cell Carcinoma 1 mg/kg IV with nivolumab 3 mg/kg IV every 3 weeks for 4 doses, followed by nivolumab monotherapy
Colorectal Carcinoma 1 mg/kg IV with nivolumab 240 mg or 3 mg/kg IV (weight-based) every 3 weeks for 4 doses, followed by nivolumab monotherapy

Pharmacology
Belzutifan is a hypoxia-inducible factor-2 alpha (HIF-2α) inhibitor that binds to HIF-2α and blocks its interaction with HIF-1β, leading to reduced transcription of HIF-2α target genes associated with cellular proliferation, angiogenesis, and tumor growth.
Ipilimumab blocks CTLA-4, a negative regulator of T-cell activity, thereby augmenting T-cell activation and proliferation including tumor-infiltrating T-effector cells and reducing T-regulatory cell function.
