Completed applications may be submitted online, via mail (Good Days Attn: Enrollment 2611 Internet Blvd, Suite 105, Frisco, TX 75034), or fax (2145703621)
A copy of insurance cards is required. Once the application is received, patients can call 8779687233 to check on the status. If patient qualifies and funding is available, financial assistance will be provided from the date of funding approval through the remainder of the calendar year or upon exhaustion of funding
Patients approved during the first three quarters of the year (JanuarySeptember) must reapply to be considered for funding in the next calendar year. Patients that are approved during the last quarter of the year (OctoberDecember) do not have to reapply and will be automatically enrolled for funding in the next calendar year. Enrollment for the following calendar year does not guarantee funding availability. New copies of household income documentation are required for each new enrollment
