•Maximum benefit per patient is twentyfive thousand dollars ($25,000) during a calendar year; patients remain responsible for remaining costs
•Program benefits must be renewed every calendar year (January 1 through December 31)
•Program may cover patient’s outofpocket costs incurred for Product that was administered up to 180 days prior to the date the patient is enrolled within the same calendar year
