Program Details

•Maximum monthly savings up to $100 per 1month fill, $200 per 2month, $300 per 3month fill
•Annual maximum savings $1,000 per calendar year
•Maximum 10 prescription fills per calendar year
•Must be enrolled in a commercial drug insurance plan covering Foundayo
•Government beneficiaries excluded (Medicare, Medicaid, Medicare Part D, Medicare Advantage, TRICARE, VA, DoD, Medigap)
•Must be a US resident (including Puerto Rico) age 18 or older
•Card activation and HIPAA authorization required
•Card expires December 31, 2026
