•$199/month for 2.5 mg dose
•$299/month for 5.5 mg and 9 mg doses
•$299/month for 14.5 mg and 17.2 mg doses when refilled within 45 days; otherwise $349/month
•Maximum savings up to $520/month on 0.8 mg, $470/month on 2.5 mg, $370/month on higher doses
•Up to 10 prescription fills per calendar year
•Must not seek reimbursement from any thirdparty payer
