•Please note that this fund may cover offlabel drugs
•This fund is for costs associated with travel
•For assistance with transportation costs related to receiving treatment, patients will be asked to provide the Foundation with diagnosis verified by a physician, nurse practitioner, or physician's assistant's signature
•Driving distance between patient's residence and drug distribution site (pharmacy or infusion center) or blood monitoring lab site must be at least 25 miles (50 miles roundtrip), based on the most direct route
•Car mileage will be reimbursed at the IRS Standard Business Mileage rate, and parking fees will be reimbursed with submission of receipts
•If patients use public transportation or hire a taxi, transportation costs incurred must be at least $10.00 one way ($20.00 roundtrip), based on the most direct route
•Proof of Date of Service is required if patient does not have a current Gout copay grant with HealthWell Foundation (an associated Date of Service payment must be processed if the patient has a current Gout copay grant with HealthWell Foundation)
•For assistance with lodging costs, driving distance between patient's residence and drug distribution site must be at least 100 miles (200 miles roundtrip), based on the most direct route
•Hotel room costs will be reimbursed up to $150.00 a night per infusion with submission of receipt
•Proof of Date of Service is required if patient does not have a current Gout copay grant with HealthWell Foundation (an associated Date of Service payment must be processed if the patient has a current Gout copay grant with HealthWell Foundation)
