•Valid only for residents in the US and Puerto Rico

•Must be 18 years of age or older

Program Details

•Eligible commercially insured patients may pay as little as $10 outofpocket expense for each 30day prescription

•Card will then be applied toward any remaining outofpocket expense up to a maximum of $600 per month

•If patient's total outofpocket pharmacy bill exceeds the maximum cap, patient will be responsible for the additional balance

•Patient can enroll and activate card online

•Card can be printed for immediate use by presenting to pharmacy with prescription
