•Valid only for residents in the US and Puerto Rico

Program Details

•Eligible commercially insured patients may receive free medication for up to 36 months (up to 12 months coverage for residents in Massachusetts, Minnesota, and Rhode Island) until the patient's insurance covers the medication

•Appeal of any prior authorization denial must be made within 90 days or as per payer guidelines, to remain in the Program

•Eligibility will be reverified on a rolling 12month basis from the patient’s first shipment date, and may be reverified at other times during Program participation

•Medication will be dispensed in 30day prescriptions
