Program Details
Enrollment process begins with referral to program by physician & faxing the completed start form to: 18556637284
Program will help patient find an insurance plan that provides coverage for the medication and will refer patient to a nonprofit organization that can help with the insurance premiums
While the patient is awaiting coverage to begin, the program will provide medication free of charge
If patient is not eligible for any insurance coverage, the program will provide medication free of charge to the patient
Patient will be reassessed annually for eligibility for the program
