With this program, eligible patients can pay as little as $0 per prescription and enjoy substantial savings. The Xospata Patient Savings Program is an excellent opportunity for patients with private insurance to manage medication costs efficiently, provided they reside in the United States, Puerto Rico, Guam, or the Virgin Islands.

Program Details
•Eligible patients pay as little as $0 per prescription
•Patient can enroll online at http://activatethecard.com/XOSPATAsavings; copay card/benefit information will be emailed to patient following enrollment
•Patient can call Xospata Support Solutions 8552213493 (Monday–Friday, 8:00 AM–8:00 PM ET); a Support Solutions Case Manager will answer any questions
•Case Managers will look up insurance coverage for XOSPATA to see if patient's insurance has any coverage requirements or restrictions, and will identify any costsharing responsibilities through insurance plan
•Patient will be enrolled in the program for a 12month period
•New benefit change includes a 2step benefit process, depending upon patient's deductible and insurance plan coverage
•1. Patient provides copay card information to specialty pharmacy; if copay card doesn't cover the entire amount owed, then
•2. Patient provides Xospata debit card information to specialty pharmacy to cover remaining amount owed.
•3. For each refill moving forward, patient should always have the pharmacy use the Copay Card information in Step 1 first
