Program Details

•Eligible patients can pay as little as $35 for XYREM for a 30day fill, subject to monthly and annual maximum benefit

•Commercial insurance only; valid for outofpocket expenses, including copay/coinsurance/plan deductibles exceeding $35 (for XYREM) per 30day prescription fill

•The Coupon Program is for patients enrolled in the XYREM REMS program

•Eligibility is assessed as part of the standard benefit investigation process performed by the pharmacy Reimbursement Specialist; no enrollment required

•Any Coupon benefit will be automatically deducted from the patient’s copay or other outofpocket cost with a patient’s verbal approval
