Program Details
•Covers copays, coinsurance, and deductibles for obesity medications
•Specifically covers Wegovy, Zepbound, and Imcivree
•Household income must be at or below 500% of Federal Poverty Level
•For patients with Medicare, Medicaid, or Military Benefits insurance
•Fund opens and closes based on available funding
•Patients begin receiving funding immediately upon approval
•Approved patients must have diagnosis and treatment verified within 30 days
•Income verification required; proof of income documents may be requested
•Grants with no claims for 120 days will be forfeited
•Patients who forfeit must wait 12 months to reapply
•Foundation covers FDAindicated drugs and offlabel if insurance covers it
