Indications

Dosing
Menopausal syndrome, Postmenopausal atrophic vaginitis EstroGel 0.06% 1.25 g (0.75 mg estradiol) once daily applied as a thin layer over the entire arm on the inside and outside from wrist to shoulder.
Menopausal syndrome Start with 0.5 mg estradiol/100 mg progesterone orally once each evening with food; adjust dose based on clinical response; attempt to taper or discontinue at 3 to 6 month intervals.
Contraindications
- Abnormal genital bleeding of unknown etiology
- Current or history of breast cancer
- Current or history of estrogen-dependent neoplasia
- Active DVT, PE, or history of these conditions
- Active arterial thromboembolic disease (e.g., stroke or MI), or history of these conditions
- Known anaphylactic reaction, angioedema, or hypersensitivity to EstroGel or any excipient
- Hepatic impairment or disease
- Protein C, protein S, or antithrombin deficiency, or other known thrombophilic disorders
- Abnormal genital bleeding of unknown etiology
- Breast cancer or history of breast cancer
- Estrogen-dependent neoplasia
- Active deep vein thrombosis (DVT), pulmonary embolism (PE), or history of these conditions
- Active arterial thromboembolic disease (stroke, myocardial infarction) or history of these conditions
- Known anaphylactic reaction, angioedema, or hypersensitivity to BIJUVA
- Hepatic impairment or disease
- Known thrombophilic disorders, such as protein C, protein S, or antithrombin deficiency

Pharmacology
Estradiol is the principal intracellular human estrogen, acting through binding to nuclear receptors in estrogen-responsive tissues; it modulates pituitary gonadotropin secretion via a negative feedback mechanism to reduce elevated LH and FSH levels seen in postmenopausal women.
Estrogen-progesterone combination; estradiol binds nuclear estrogen receptors to reduce elevated gonadotropin levels and maintain secondary sexual characteristics in postmenopausal women, while progesterone binds progesterone receptors to transform proliferative endometrium into secretory endometrium and oppose estrogen actions.
