Dosing
Chronic pain syndrome Starting dose 20 mg orally once daily for opioid-naive patients; titrate in increments of 10-20 mg every 3-5 days; doses >=80 mg reserved for opioid-tolerant patients only; initiate at half the starting dose in severe hepatic impairment or moderate to severe renal impairment and end-stage renal disease.
Chronic pain syndrome Adults not opioid tolerant: initiate 10 mg orally every 12 hours; opioid-tolerant adults: individualize dose with titration every 1-2 days; pediatric patients 11 years and older already tolerating >=20 mg/day oxycodone equivalent: convert from prior opioids per established conversion factors; geriatric patients (debilitated, not opioid tolerant) and hepatic impairment: initiate at one-third to one-half the recommended starting dose and titrate carefully.
Contraindications
  • Significant respiratory depression
  • Acute or severe bronchial asthma in an unmonitored setting or in the absence of resuscitative equipment

Pharmacology
Hydrocodone is a full opioid agonist with relative selectivity for the mu-opioid receptor; it produces analgesia via CNS opioid receptors in the brain and spinal cord, and also causes respiratory depression through direct action on brainstem respiratory centers, reduced GI motility, and peripheral vasodilation.
Oxycodone is a full opioid agonist relatively selective for the mu receptor that produces analgesia through central opioid receptor activation with no ceiling effect on analgesia; it also causes respiratory depression via direct action on brain stem respiratory centers, miosis, GI motility reduction, and peripheral vasodilation.
